Abdominal Aortic Aneursyms Essay

Aneurysms were first described by the 16th century anatomist and physician Vesalius, who believed they were simply a widening of the vessel (Collin et al 2009). An abdominal aneurysm (AAA) is a condition in which the abdominal aorta (a large blood vessel that supplies blood to the abdominal, pelvis and the lower limbs) becomes large and ballooning leading to the development of several symptoms. The condition more often occurs in males compared to females. It occurs more frequently in above the age 60. When the aortic aneurysm is larger than size, it is more likely to rupture causing life-threating problems. This is a medical emergency requiring critical care. This complication is present in about 20% of the people affected with AAA. Another complication with AAA is aortic dissection in which the innermost membrane of the blood vessel ruptures due to the intense pressure causing blood to be filled within the wall of the artery. Symptoms of the condition usually develop suddenly due to rupture of the wall or breakage of the innermost wall. Sudden rupture of the abdominal aortic aneurysm, often without prior medical warning, is the 13th leading cause of morality in the US (Li, Z 2006). Some of the symptoms that can develop in AAA include:- * Abdominal pain (which may be severe, consistent and radiates to the legs, groin and the buttocks region) * Pulsations in the abdomen & palpitations * Nausea and vomiting * Anxiety and agitation * Abdominal rigidity * Cold and clammy skin * Presence of the abdominal mass * Excruciating pain in the limbs and back, when the AAA ruptures * Fatal outcomes in the case of ruptured AAA(Albright JL. 2006 & Hallett JW. 2008) When the AAA is small in size (less than 5 centimeters), no treatment is required. Antihypertensive may be needed to prevent any further complications from developing. Besides, if the individual has any risk factors that can worsen the condition, it needs to be rectified immediately (such as giving up smoking, weight reduction, lowering cholesterol levels, etc.). Periodic evaluations have been recommended to ensure that any risk can be identified and immediately taken care of (Albright JL. 2006 & Hallett JW. 2008). Surgery is required if the AAA is larger than 5.5 centimeters in size, as the risk for rupture or dissection is present. The Aneurysmal defect is repaired by inserting a surgical graft. This can be performed by two method namely the conventional approach or the endovascular approach. In the conventional approach, general anesthesia is utilized. A surgical incision is made below the breast bone, the aneurysm defect identified, and the graft material sutured in position. The entire procedure takes about 5 hours and a stay of at least a week in the hospital is required. The second approach is the endovascular stent grafting in which regional anesthesia is administered and a catheter is introduced through the femoral artery present in the groin region. This catheter contains the stent graft. It is gradually guided into position using imaging techniques. Once it is position, the stent graft is opened ensuring a stable blood flow. The procedure takes about 3 hours can require a stay of about 3 days in the hospital (Albright JL. 2006 & Hallett JW. 2008). Several imaging techniques play a very important role in diagnosing and treating AAA. These include ultrasound, CT scans and angiography. Abdominal ultrasound is one of the preferred examinations for AAA. Ultrasound of the abdomen is also required following convention surgery to monitor the repaired AAA closely. It is usually performed as an initial imaging modality due to several factors including:- * Portability * Absence of ionizing radiation * Low costs * Easy availability (Radvany MG. 2006) Angiography involves administration of a contrast media into the femoral artery present in the groin region and then taking X-rays to determine the condition of the abdominal aorta. It is very useful before conventional and endovascular surgery for planning. It also seems to be very useful in the case of aortic dissection. However, angiography also carries a few risks including:- * Damage to the artery * Hypotension * Infection of blood vessel * Embolism and clot formation * Bleeding and heart attack (Bentley-Hibbert S. 2007 & Radvany MG. 2006). If the abdominal ultrasound and AAA greater 5 centimeters, than a CT scan of the abdomen is required. The CT scan can better help to plan the surgical intervention as the images provide a lot of detail (including involvement of the renal arteries, size of the aneurysm, amount of calcification, presence of mural thrombi, etc.). The accuracy of CT scans is said to be 100%. They provide a lot of details regarding the size of the aneurysm and also about distal and proximal issues. CT even with contrast media cannot be utilized to study dissection aneurysm or the presence of the extent of mural thrombus. They are also required following endovascular graft surgery for a period of 6 months as a post-procedural follow-up measure (Radvany MG. 2006). MRI scans of the abdomen are required when the side-effects of the contrast media used in other techniques could be potential damaging to the patient (in case of kidney or liver problems) or when radiation is contra-indicated. The images provide a lot of detail and are accurate. However, MRI cannot be performed in individuals with cardiac pacemakers. The Instincts approach is said to be a set of skills, preprogrammed in people and animals. These skills are biological in nature and programmed into our DNA. Instinct is not something they can be taught, it's an energy that guides various species in specific directions. Some of instincts drive us to reproduce, to birth and raise our children a certain way.D rive-reduction approaches to motivation that energize the behaviors to fill a need. For instance, when you're thirsty the need to drink, or to eat, sleep and we produce. There are also drives that leads us to want to learn or achieve things. Arousal approaches motivates us as humans, to have a certain amount of activity and stimulation. This could be socializing or doing activities and bring out a higher level of arousal. Incentive approaches to motivation the desire to obtain a goal or incentive.This incentive loud be four love, affection, food, sex or another particular goal. Cognitive approaches to motivation is the drive for your own personal enjoyment and satisfaction, rather than a reward that is tangible. This could be motivation of personal pride and satisfaction that physical things may not accomplish. There are five main emotions that drives us: we have love, Joy, anger, sadness, and fear. Love, can give us all of these emotions, when you love someone it brings you Joy.And this love can also cause us to feel sadness, fear and anger. We may worry that somebody will not love us back and this can bring the sadness. You could also be Jealous or angry if somebody does not love us back. Meanwhile, there is one positive emotion about being a returning student here the University of Phoenix and that emotion is pride. As a returning student in my forties, I have so much pride in myself and I know I will not give up on my education. Within a year I will have my degree and no one can take that away from me.This is something that, I Alone, have put in the hard hours for my own personal success. On the other hand, the one negative emotion about returning to school is worry. I worry about everything, from the roof over my head to feeding my family. But, I worry more about school, my grades and completing my assignment of on time. I worry that I may not have what it takes to be a college graduate and do not have the strength to complete my degree. I worry about suc ceeding as a student. The incentive approach to motivation is what will motivate me to complete my degree.Each week I seek out the results of my grades s an incentive and the fulfillment in my motivation to complete my college degree. This incentive helps push and pull me through to the next week. Every week that is completed, is one week closer to achieving goals. The grass will withstand heavy grazing and so was introduced for this purpose so the conversion of forest, homogenous grassland and savanna to pasture for grazing created the ideal environment for this hardy grass to grow. However the problem lies in that this invasive grass has spread from cultivated areas to natural ecosystems at an alarming rate as it will grow in all soil types and including sandy and stony soils. Bufflegrass grows densely and so crowds out native plants of similar size and due to its ability to resist drought will remain dense even in dry years. It attacks native plants by its ability to win competition for natural resources such as water, space and soil nutrients so prevents homogenous plants from maturing and germinating and secondly it is highly flammable but resistant to fire, once a grass fire has occurred it is not unusual that only the Bufflegrass grows back. The Sonaran Desert evolved without fire as an ecological factor and most of its plants cannot tolerate it. The Bufflegrass populations in Saguaro National Park have quadrupled in size between 2002 and 2006 despite the outbreaks of several vastly spread fires (Bean & Betancourt, 2006). The economic factor is that with the increased risk of fires utility infrastructures are threatened, property may decrease in value as areas become known for fire hazards, insurance rates rise not to mention the cost to governments in having to find means of managing the invasive grass. Being an animal exclusive to Australia as well as one that can’t move backwards, the kangaroo is symbolically appropriate in representing Australia and the countries progress forward. Historical, Social, Cultural Context The kangaroo holds a significant historical, cultural and social background to Australia. Before European settlement (around 1780’s), the kangaroo was an important animal to Aborigines for its meat, hide, bones and sinews. It was also used in Aboriginal dreaming stories (also an icon of Australia). There is a well-known myth about kangaroo’s and how they got their name. When European settlers arrived, seeing such a strange animal they asked the Aboriginals what it was, and they replied with ‘kanguru’. In their language this meant ‘I don’t know’ which is supposedly how the kangaroo got its name. Aboriginal tribes have many different legends regarding the kangaroo and some see it as a reincarnation of their ancestors. Over the past 200 or so years, the kangaroo has broadened a lot in historical significance; it is one of the oldest icons of Australia and therefore should be kept that way. By contributing the Kangaroo to a National Australian exhibition, it will help to keep the icon of the kangaroo alive and significant. The kangaroo represents Australia’s cultural and social background and is internationally recognisable. It represents the Australian character in many ways including: Its size, strength and speed, which make it a national logo/emblem for Australian organisations and especially sporting clubs. Kangaroo’s are also symbolic of another Australian trait; to stick up for your self and not back down. With their large feet and long tail kangaroos find it hard to move backwards, indicating that Australians are people who are moving forward and growing as people of Australia. The Kangaroo is so well represented as Australian that it has come to support and embody the dominant Australian discourse of both indigenous and non-indigenous people of Australia. This is evident through strength, fighting spirit, individualism, attitude, and their tough/rough personalities. To conclude this proposal, the kangaroo should be contributed to the National Exhibition titled ‘Icons of Australia’ because it represents Australians in such a way that it is known by most people throughout the world. The Kangaroo is recognised culturally, socially and historically and symbolises Australian discourse through its many traits. It is a unique animal to Australia and makes a huge contribution to Australia’s national identity. Representing Australia in so many ways, the kangaroo plays such a big part in Australia’s culture, history and even future, the National exhibit would be such a good way to show and help Australians understand how such an icon can play such a big part in the country of Australia. Today, some companies do hand-painted shoe business, for example, Converse was very famous for its canvas shoes. In recent years, Converse put out hand painting shoes, but these shoes are limited. So this cannot satisfy most customers demand .Our hand-painteg shoes are having a rational price and when the customer want these shoes we can painted for them. Most people can buy a pairs of shoes they favour and show their individual. 2. Internal analysis Our on-line shop sell hand-painted canvas shoes, but for different customers we also sell the colour pen and no pattern shoes, so customers can paint the shoes in any way they like. For all the customers we will teach them how to wash the shoes and they can return of goods if shoes have any problem within three days. Because I have 4 physical shops in high street, so I have existing customers and I have enough money to set up my special online shoe shop. Start my business I need some people to help me. If the factory makes the mistake of the shoes or do not give us the raw shoes on time, we will face trouble, for example, we can not give the hand painting shoes to customer on time or the shoes have quality problem, these will make us lose trust from customer. So when the customers order the shoes, we can paint the pattern they like on shoes and send them to customers at s short time. If we have enough stock, we can save much time and can attract more customers. Information flow Information will affect the sellers decision. All the information we get on Internet. E-commerce technology gives a fast way to communicate and share information. Every time have its own need, the seller must understand what the customers need and catch the opportunity. We can build an information system and get the information from the internet. For example, we can ask the customers do the questionnaire online, so we can know the customers demand. Depends on the information we will do the right products and service. Special shoes can use an Information System that will help them know where deliveries are; what customers are buying; where customers are; what the best prices are for their suppliers. They can have a Just-In-Time system to use this. That way they can use the information flow to bring better service to the customer and make lower costs to make prices lower. E-Procurement We do not have our own firm to produce the raw shoes, so we must purchase these raw shoes from the firm and for our hand painting shoe, we need purchase the pigment and the painting brush. This is B2B system and we finish the procurement on Internet. E- Procurement helps us purchase the raw products quickly and reduce the error.
